Overview

Students can use this form to ask that their grade be reviewed and changed, if possible.
 

Instructions

Click "Grade Appeal.PDF" from Attachments (1) section on this page to download. 

• Edit/Complete the fillable PDF form and sign.
• Forward form for required signatures.
• When completed, the student will be notified of the outcome.
